Job Description :

Patients First Home Care LLC is looking for a Full Time Physical Therapist to provide in-home visits to our patients.

Provide a comprehensive patient examination/evaluation. Complete Oasis assessments timely and accurately. Reassess patient every 30 days and make necessary revisions to the plan of care in conjunction with the physician and documents the patient's continual needs and progress.

Provides patient oversight and supervision of Physical Therapy Assistants. Supervision of Home Health Aides with updates to care plan as needed.

Participates in patient care conference, updates and assists other members of the care team. Participates in; in services, chart audits, quality improvement activities.

Qualifications

Current license to practice Physical Therapy in the State of Florida

Graduated from an accredited physical therapy educational program

One year of physical therapy experience. Preferably 1 year home health experience

Current valid Florida Driver's License with current vehicle insurance

Current CPR certification

Job Type: Full-time

Pay: $90,000.00 - $110,000.00 per year
