Position Overview:

To ensure the delivery of integrated, comprehensive medical services to Ryan-NENA's HIV/AIDS population, assuring that HIV/AIDS infected individuals receive the highest quality of case-managed care throughout all areas of the Center. Assume administrative responsibility for the collection of data and submission of all required programmatic statistics and reports. Maintain a panel of HIV Primary Care patients. This work is carried out in support of the mission and goals of Ryan Network.

Essential Functions:

  • Provide comprehensive clinical case management services to all HIV positive patients enrolled in Ryan-NENA's HIV Primary Care Program. Maintain progress notes for clinical case management appropriate to patient's medical and psychosocial needs. Follow-up on abnormal reports, lab findings, and missed appointments, both at the Center and for off-site specialty referrals.
  • Facilitate clinical services as mutually agreed by the Internist, Pediatrician, and other physicians as outlined in medical protocols, including assisting the physician with an examination of patients, as needed.
  • Coordinate clinical and psychosocial needs of the HIV positive patients, in conjunction with the Medical Provider, Case Manager, and Social Worker, in areas of home care, hospitalization, and discharge.
  • Monitor LPN provision of care to HIV patients, and oversee their activities and duties in conjunction with the Medical Director and appropriate Clinical Unit Coordinator.
  • Visit hospitalized Ryan-NENA patients as needed; maintain an ongoing relationship between Ryan-NENA and the patient to encourage continuity of care.
  • Provide information, at the patient's request and with informed written consent, to medical providers, family members and significant others regarding the patient's health status and other special needs.
  • Serve as an advocate for HIV positive patients in all areas of concern in conjunction with the patient's primary care provider and Mental Health Department.
  • Ensure the Program's compliance with all HIV confidentiality policies and guidelines, as well as all clinical protocols and policies and procedures related to HIV clinical case management.
  • Ensure the effective coordination of services with the Mental Health Department.
  • Serve as co-facilitator of weekly Case Conference meetings.
  • Maintain a panel of HIV Primary Care patients in accordance with the Protocol and Standards of the Center's mv Primary Care Program and the AIDS Institute of the State of New York, Department of Health.
  • Collect all pertinent patient data upon intake into the HIV Primary Care Program, collect and maintain current data and health information, and record all information in the patient's medical record.
  • Collect the necessary data in collaboration with the primary care provider, Clinical Unit Coordinator, and Mental Health practitioners in order to make prospective decisions in the patient's comprehensive care and treatment plans, e.g., hospital care, hospice care, etc.
  • Assist in the coordination and maintenance of the Center's HIV confidential computer database. Work with the Database Manager to ensure all appropriate patient data is entered into the database.
  • Provide medical care to adult patients with acute or subacute medical problems. This responsibility includes the recognition of acute symptoms, management of emergencies, detection of abnormal findings on physical assessments, reporting of findings, and clinical decision making. In collaboration with the supervising physician, provide routine medical treatment based on policies, protocols, and practice agreements appropriate for licensure for treatment of acute medical problems. Provide appropriate information and education to patients.
  • Provide patient services and back-up coverage for other providers.
  • Facilitate the appropriate transfer of patient information to and from the Center in a timely and confidential manner.
  • Assist in the generation of reports required by funding sources as directed by the Planning Department or the Director of Services, and in the preparation of grant proposals, as needed.
  • Assist in the coordination of HIV in-service education and training efforts in conjunction with the Director of Special Services and the Medical Director.
  • Attend all supervisory and other in-house administrative and medical meetings as assigned; attend meetings related to HIV services, as requested.
  • Facilitate the development and ongoing relationship with other healthcare organizations and institutions providing specialized care to participants of the Center's HIV Primary Care Program.
  • Under the direction of the Medical Director, coordinate, supervise and participate in chart audits for HIV positive patients for quality purposes, including the tabulation of results and distribution of reports. Participate in the development, implementation, and assessment of plans of connection as needed.
  • Actively participate in the Center's Coordinating Committee for HIV Services and in the Quality Improvement Committee.
  • Participate in Evening and Saturday administration coverage, and in the administrative beeper rotation, as assigned.
  • Assume responsibility for professional growth and development by participating in professional continuing education, attending in-service seminars and training, reviewing current professional literature, and maintaining membership in professional organizations.
  • Attend all meeting and/or training sessions as required.
  • Immediately report any problems or unusual occurrences to a supervisor.
  • Practice accepted infection control measures in accordance with the OSHA Bloodborne Pathogens Standard, and Center policies and procedures.
  • Perform other duties as assigned.

Minimum Experience and Skills Required:

  • Three (3) years of experience in a clinical setting and two (2) years of administrative experience.
  • Supervisory experience in an ambulatory, public or community health setting.
  • Knowledge of HIV/AIDS issues, treatments, and services.
  • Excellent oral, writing and computer skills.
  • Experience with HIV infected population.

Education, Licenses and/or Certifications Required:

  • MS in Nursing; Physician Assistant: BA/BS.
  • Current New York State Licensed Nurse Practitioner or current New York State Licensed Physician Assistant.

Preferred Qualifications:

  • HIV Counseling Certification.
  • Bilingual Spanish.
