At least a bachelors degree in social work from a school of social work accredited by the Council on Social Work Education. This position is paid per visit.

Develops appropriate care plans in coordination with the patient/family, provides social work services as indicated, and assesses social work and team services

Effective communicator and active participant in morning huddles. Primarily responsible for covering routes within a territory. $1,000 Sign-on bonus.

Active and current PT license. Visit a variety of clients in their own home, helping them recover from an injury, illness, surgery, or hospitalization.

Candidates must be comfortable with developing strategies to keep track of various tasks and requests while also addressing interruptions from patients,
