Home Health Occupational Therapist (OT)
Pay will be $75-$130 per visit to the patient.

We are looking for an Occupational Therapist to care for our patients from the comfort of their homes and facilitate their health. You will be responsible for providing skilled occupational therapy services, on an intermittent basis, to patients in their homes. The goal is to promote the patient’s well-being by providing high-quality OT care, maximizing their quality of life.

Responsibilities

• Monitor patient’s condition and assess their needs to provide the best possible care and advice
• Provide skilled occupational therapy care as outlined in the patient care plan
• Evaluate the patient, the patient's significant other(s), and the home situation to determine what instruction will be required, what assistance will be available from the family in caring for the patient, and what other community services will be required
• Observe and interpret the patient’s symptoms and communicate them to the immediate supervisor when needed
• Submit clinical notes, progress notes, and other clinical record forms, outlining the services rendered, as indicated
• Help improve the client by guiding and instructing them in prescribed therapeutic self-care and creative activities directed towards improving independence and physical and mental functioning
• Instruct patient in care, and use, of wheel chairs, braces, splints, and prosthetic and orthotic devices
• Teach and counsel the family and patient in the total Occupational Therapy program and other related problems of the patient at home
• Participate in the patient's discharge planning process
• Adhere to federal, state, and accreditation requirements, including Medicare and Medicaid regulations
• May be requested, by Clinical Manager, to fill in for the other OTs

Skills

• Experience as an occupational therapist (preferred, but not required)
• Excellent knowledge of OT care methods and procedures
• In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination, etc.) and willingness to follow them at all times
• A team player with excellent communication and interpersonal skills
• Responsible and compassionate
• Outstanding organizational and multi-tasking skills
• Patient with excellent problem-solving skills

Qualifications

• Successful completion of an Occupational Therapist education program
• Must have a valid occupational therapist license
• One or more years of experience in community/home health agency or in a hospital setting (preferred, but not required)
• Current CPR certification
• Ability to keep up with strenuous activity, such as walking, sitting, stooping, standing, and minimal to maximum lifting of patients, including the turning of patients
