Hiring Annual Salary Range: $69,600 - $81,000
This position can work from home or office. Home visits are required.
Sandhills Center is a Local Management Entity-Managed Care Organization (LME-MCO) serving
people in nine counties in central North Carolina. We have a robust network of providers who offer
treatment and support for mental health, intellectual/developmental disabilities and substance use
disorders.

Duties:

• This position will provide occupational therapy assessments and input for Complex Care

Coordination.

• Will provide support for adult members that are medically fragile or have significant chronic

health conditions, have a mental health, substance use or co-occurring disorder who are
transitioning out of Adult Care Homes (ACH) into the community.

• Individuals served may also have a co-occurring intellectual or developmental disability.

• This position will work collaboratively with staff, behavioral health providers, Primary Care

Physicians, specialty care providers and other community partners and stakeholders to
support members in their home communities.

Minimum Qualifications: Master’s degree in occupational therapy; or, Bachelor’s degree in
occupational therapy with two (2) experience working with the co-occurring physical health and
behavioral health individuals.
Licensure Required: Licensed to practice as an Occupational Therapist in the State of North
Carolina.
Other Qualifications: Must have a valid driver’s license and reside in NC.
