FULL JOB DESCRIPTION

Preferred Transitions is looking for Physical Therapists to join our 5 Star agency. We mainly service Worcester, Norfolk, Bristol and Middlesex Counties and are looking for clinicians who are committed to provide exemplary patient centered therapy to the Medicare population. We are a dynamic VNA with “an old fashioned VNA feel” who put patient care first.

PHYSICAL THERAPIST JOB DESCRIPTION

-Provide initial, ongoing, comprehensive assessments of the patient’s needs including all OASIS time points

-Assist in Development and implementation of the interdisciplinary patient care plan

-Provide standardized testing, functional assessments and be able to create a patient centered plan of care

-Supervise Physical Therapy Assistants and home health aides as per agency policy

-Communicate with care team including all clinicians/members involved in patient care

-Participate in case conferences, staff meeting, and in-services programs per agency policy
