Salida Del Sol ADHC is a Community Based Adult Services (CBAS) licensed program that provides services to eligible older adults and/or adults with disabilities to restore or maintain their optimal capacity for self-care and delay or prevent inappropriate or personally undesirable institutionalization.

RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:

1) As a member of the multidisciplinary assessment team, will evaluate the participant's rehabilitation potential. An individualized treatment plan will be developed and modified as needed, based on change of status.

2)Will reassess participants receiving Physical therapy services at least every six months and update the plan of care.

3) Record and sign daily treatments in the participant's health record including the

reaction of the participant to the treatment given.

4) Record, sign, and date quarterly progress notes.

5) Review and initial all evaluations and the discharge summary.

6)Develop maintenance program treatment plans and train personnel that will be providing

the service and the nurse who will be supervising the provision of the maintenance program.

7) Interface with other team members, the participant's personal physician, referral agencies, and significant others when appropriate.

8) Carry out appropriate assessment and treatment interventions within scope of practice.

9) Comply with all health and safety standards including licensing, fire and disaster preparedness regulations and agency policies.

10) Conform to all applicable policies of the Center and attend the orientation program offered by the Center.

QUALIFICATIONS

Graduate of an accredited school of physical therapy and certified by the Physical Therapy Association. Prefer previous experience in working with functionally impaired older adults in a long- term care setting. This contracted position reports to the Program Director.

6-10 hours per week.
