Job Description :

We are a Medicare certified skilled home health agency seeking home health Physical Therapists. Our services include skilled nursing, physical therapy, occupational therapy, speech therapy, and medical social work.

Job Description:

A Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Nursing Supervisor.

  • Selects evaluation and treatment methods appropriate to the diagnosis, age group and patients physical and emotional status and formulates appropriate treatment plans.
  • Demonstrates essential skill competencies in the use of treatment modalities, equipment and protocols.
  • Performs re-evaluations according to department guidelines, adjusting the treatment plan accordingly.
  • Adheres to department policy regarding timeframe, form and content of written evaluations, progress notes and discharge summaries.
  • Communicates effectively with patients, family, medical staff, care team members, students, supervisors and outside contacts.
  • Completes all administrative and other responsibilities in support of the operating unit.

Qualifications:

  • Currently licensed in the state of Virginia.
  • Ability to exercise initiative and independent judgment.
