Clarity Clinic is a 5-location interdisciplinary group private practice of psychiatrists, psychiatric PA's, Psychologist (post Docs), and Therapists. Our mission is to thoughtfully guide the whole person on their journey to find clarity and mental wellness by providing exceptional holistic care.
Our staff of Psychiatrists, Physician Assistants and Psychotherapists offers the latest medication and therapy treatment as we help guide our clients towards mental wellness and a balanced personal and professional lifestyle. This multidisciplinary approach allows us to provide holistic care in psychiatry, psychotherapy and much more. Responsibilities

• Conducts psychiatric evaluations, medication assessments, and crisis response to new and existing clients.

• Develops individual service plans based on assessment data, and provides therapeutic interventions as necessary including the provision of on-going medication management services.

• Provides consultation and health education to agency staff, caregivers and clients as it is related to the treatment and medication needs of the center.

• Maintains documentation of services including service plans, progress notes, medication informed consent, and other required information in compliance with the company and third-party compliance standards.

*Qualifications*

• Possession of current license to practice as a Psychiatric NP/PA as granted by the State of Illinois
• Cultivate a climate of trust and compassion for the patients
• Comply strictly with medical care regulations and safety standards,
• Document medical consultations, intakes, medical procedures and progress notes accurately in a timely fashion
• Issue appropriate prescriptions and administer medicine
• Inform physicians about any problems or discrepancies that might arise
• Collaborate effectively with team members of various specializations

Conditions of Employment

• Must pass a pre-employment criminal background check.
• Subject to credentialing process; must maintain credentialed status, as applicable.
• Must maintain certification status.
• Must maintain licensure status.
