The nursing Unit Supervisor is responsible for the coordination/implementation of resident care as directed by a multidisciplinary care plan and supervision of nursing personnel for an assigned area.
A. Education and Training - Current New York State Professional Nurse registration; preferably a B.S. in nursing. Current cardiopulmonary resuscitation (CPR) certification issued by either the American Heart Association or the American Red Cross. Interest in developing nursing and management abilities and furthering education in these areas through membership in professional organizations and/or participation in workshops and seminars.
B. Experience - Previous experience as a Charge Nurse or Supervisor in long term care is preferred.
C. Character - Must possess good moral character, compassion for the elderly, have the ability to supervisor licensed and allied nursing personnel, and create/maintain good staff relationships; must maintain all information in the strictest of confidence.
D. Interpersonal skills - must possess the ability to communicate in a calm, non-threatening manner with residents, families, volunteers multidisciplinary team and co-workers. Is an advocate for the Schofield Residence, its purposes, goals and policies.
III. Essential Job Functions
A. Read, write, speak, hear, reason and remember.
B. Stoop, kneel, crouch, reach, stand, walk, push, pull, and lift medium weight (25-50 lbs.).
C. Orient, instruct, guide and evaluate nursing personnel; to comply with standard policies and procedures as established for the facility and maintain staff awareness and compliance with these; to assess residents' conditions and appropriately report on and coordinate care planning needs on the shift.
D. Must be willing and able to perform basic life support (BLS) at all times, including CPR, the Heimlich maneuver and rescue breathing
E. Perform all job duties and responsibilities as outlined in IV.
IV. Job Duties and Responsibilities
The Unit Supervisor is directly responsible to the Assistant Director of Nursing. The job duties and responsibilities are to be performed while observing the following standards: job standard (knowledge, ability, skills), technical skill, quality, quantity, judgement/problem solving, decision making, and initiative. The responsibilities include, but are not limited to the following:
A. Introduce, observe, and supervise new procedures.
B. Consult with nursing staff regarding nursing procedures and recommend nursing policies.
C. Assume administrative responsibility in the absence of other administrative personnel.
D. Supervise adherence to care plans.
E. Arrange for or provide resident/family counseling, if necessary, and in conjunction with Social Services.
F. Participate in discharge planning.
G. Conduct staff conferences, at least monthly.
H. Provide information and/or resolve resident care problems.
I. Identify those components of the nursing regimen which may be delegated to allied nursing personnel and provide teaching and guidance when necessary.
J. Assign, re-assign staff to meet staffing needs.
K. Perform duties as listed on job description for staff nurse as necessary.
L. Interpret principles of good management to nursing personnel and implement application of these principles in daily work in cooperation with other departments.
M. Assess unit needs by making periodic rounds of unit.
N. Supervise implementation of nursing regime for assigned resident, including evaluation and revision of regime as indicated.
O. Communicate to physician, family or designated representative, condition changes occurring with the resident or significant events r/t the resident, in a timely fashion.
P. Evaluate emergencies and determine emergency measures to be used; give or direct emergency treatment required by residents or personnel; notify physician in the case of accidents; complete incident/accident reports.
Q. Counsel/discipline nursing personnel when necessary including verbal and written warnings, post suspension follow-up, in conjunction with the Assistant Director of Nursing and the Director of Nursing.
R. Collect statistics, prepare and submit records as required, maintain accurate records and emphasize confidentiality of resident information.
S. Ensure the appropriate documentation of resident assessments and conditions/events.
T. Assist in establishing and maintaining good personal and public relations.
U. Assess unit needs, conditions, resources, and make changes essential to delivery of appropriate nursing care services.
V. Coordinate new employee orientation, including assessing skill proficiency, safety and competence in assuming responsibilities of position.
W. Assess employee performance with audit tools and performance evaluation, within specific time frame providing ongoing feedback to employee.
X. Maintain professional competence through participation in continuing education and other appropriate learning experiences.
Y. Perform other related duties as required by their manager.
V. General Duties and Responsibilities
A. Participate in all department meetings, mandated inservices, and assigned committees.
B. Maintain a positive and professional relationship with residents, registrants, family members, legal representatives and visitors.
C. Maintain a positive and professional relationship with all personnel, including supervisors, co-workers and volunteers.
D. Follow all safety regulations and precautions at all times; use protective devices as required.
E. Wear and maintain proper uniform/clothing as established in the uniform dress code policy, including name tag identification. Maintain acceptable personal hygiene.
F. Maintain confidentiality and resident/patient/registrant rights.
G. Maintain confidentiality of all issues and subject matters, including records, reports, data and other related sensitive materials.
H. Maintain an acceptable attendance rate.
I. Follow all laws, rules and regulations of the Federal and NYS Health Codes in regard to the department and overall organization operations.
VI. Supervisory Duties and Responsibilities
A. Plan, develop, organize, implement and evaluate the department’s work as appropriate.
B. Delegate authority, responsibility, and accountability to other responsible department personnel.
C. Provide leadership by being an appropriate role model to others.
D. Advocate the purposes, goals and policies of the Schofield Residence and affiliate organizations.
E. Provide departmental personnel with ongoing feedback, including timely evaluations of staff, counseling of employees and taking disciplinary action when necessary.
F. Provide effective supervision, ensuring all staff abides by all government laws, rules and regulations that affect patients, families, coworkers and volunteers of the Schofield Residence and affiliate organizations.
G. Use any resources available to create ideas, programs or more effective practices that will benefit the organization, department and/or staff.
VII. Goal Setting and Management Objectives
A. Attain a deficiency free NYS Department of Health survey and/or federal survey, and if required, resolve any deficiencies in a timely and effective manner.
B. Work with residents/registrants/patients, families, and legal representatives to ensure that substantiated complaints are resolved in a timely and effective manner.
C. Accomplish fiscal results within established department budget for the most recent fiscal year.
D. Make and recommend timely budgetary adjustments as indicated, by analysis of the monthly budget and payroll hours, for the most recent fiscal year.
E. Establish and complete all departmental performance goals.
F. Establish and complete all individual performance goals.
VIII. Working Environment
The worker is subject to inside environmental conditions and protection from weather conditions. May also be exposed to odors, fumes or mists from medications. Has access to an office, but primarily performs duties throughout resident areas. Hours of work may be flexible, dependent upon departmental needs, but all shifts require nursing supervision.
IX. Exposure to Blood and Airborne Pathogens
This position has been categorized into Occupational Safety and Health Administration (OSHA) Category 1: routine contact with blood/body fluids of persons living or non-living. Contact can be made while performing personal care, handling regulated medical waste and in clean up of spills and other blood/body fluid accidents. Protective equipment should be utilized when performing these tasks.
