Job Description:
Plans, coordinates, and implements Social Services for patients of all ages, patients' families, and other person significant to patients at the Medical Center. Anticipates and attends to patient's specific psychosocial needs, financial needs/concerns, equipment needs, educational needs, and community resources from admission through transfer to the next appropriate level of care. Assists with discharge planning, grief counsel, financial application, and access to community resources as needed. Enhances patient care, thus contributing to the goals and mission of the Medical Center.
Education Qualifications:

A bachelors degree in social work (BSW).

Licensure/Certification Qualifications:

Current State Social Work Licensure.

The approximate base compensation range is posted, but the actual offer will reflect our total compensation and benefits package. The offer will be determined by a number of factors including the selected candidate's experience, knowledge, skills, abilities, FTE, as well as internal equity among our team.
