VNA Care Network is currently seeking a full time (32-40 hours per week) Masters of Social Work (MSW) in our Canton/ Dedham / Norwood and surrounding towns territory. Responsibilities include: In Home Psychosocial assessment for patients. Able to complete referrals as needed and assess counseling support needs. Collaborate with team members and community agencies to support patient's Plan of Care. Assist patient's and Caregivers with obtaining services/support as needed. Complete documentation following visit in agency Electronic Medical Record.

This position requires a MSW, LCSW or LICSW. Competitive wages, excellent benefits package, and mileage reimbursement. Clinical Supervision available as needed.
