Job Description :

Position requires a Masters in Social Work or other related field.

Position is 40 hours per week, M-F with occasional supervisor on call responsibilities

  • Supervises Adult Protective Services Case Managers and Senior Care Coordinators, time frames and caseloads, including all tasks required to complete cases and reviews documentation/files in accordance to agency funding source(s) requirements, in preparation for billing, to prevent errors and to ensure accuracy.
  • Responsible for assigning work to Adult Protective Services Case Managers, Senior Care Coordinators, Case Aides and Administrative Staff and ensuring it is performed in an efficient, prompt, professional and accurate manner in accordance with programmatic guidelines and rules, and provides guidance when necessary.
  • Works independently and consistently exercises discretion and sound judgment in directing the Adult Protective Services staff to conduct investigations and protective services to victims of abuse, neglect and exploitation to elderly individuals, people with disabilities (ages 18-59) and their families following the Illinois Department on Aging and Area Agency on Aging Adult Protective Services (APS) program standards and reporting procedures.
  • Complies fully with all APS regulations and program plan requirements which may include required time frames for priority level cases, and substantiated cases, and all service activity. Utilizes a variety of clinical modalities to engage and intervene with client, family members and others involved.
  • Ensures that all duties, including the investigation of reports of abuse, neglect and exploitation are completed in accordance with applicable laws, rules, policies and procedures and also ensures that staff appropriately and skillfully engage clients, family members and others involved toward reaching a safe and suitable determination.
  • Ensures that staff document all cases appropriately by reviewing all case files in prescribed timeframes and tracks that each staff person files and required documentation are sent to funders within required timeframes. Also ensures that all case files are compliant with requirements of funders, contractors, governmental regulations, Council on Accreditation and Agency policies and procedures.
  • Demonstrates foresight in recognizing problems and a vision in identifying and implementing solutions. Ensures that programs are producing desired outcomes and program goals, and delivers services at required capacity. Informs supervisory chain of command the status of work and if meeting required timeframes and works with supervisor to make needed changes or improvements.
  • Communicates regularly through individual, supervisory and group meetings to implement Agency and programmatic goals and requirements. Provides timely and accurate feedback to each employee under his/her supervision.
  • Demonstrates the ability to analyze employee performance, is able to work with employees to improve unsatisfactory work, and identifies opportunities for growth.
  • In conjunction with the Program Director, is responsible for program budget, fiscal and programmatic reporting, grant writing, billing and strategic direction of the program.
  • Participates in community education, training and outreach.
  • Conducts regular meetings and trainings to convey information and resources to promote the professional development of staff and establish a collaborative environment.
  • Responsible for the selection, hiring, coaching, supervising, training and development, discipline and evaluation of staff performance, and ability to motivate staff to perform optimally.
  • Must be able to drive, have own car and have minimum vehicle insurance. Position requires home/community visits. Federal mileage reimbursement rate paid for business mileage incurred.
