Summary

Under the supervision of the Director of Communications, this role will provide support and education to donor families after the loss of a loved one who had the opportunity to become an organ or tissue donor. Develops robust programs to meet the needs of donor families and demonstrate We Are Sharing Hope SC’s commitment to the care of donor families while the honoring the legacies of donor heroes. Through progression of planned contacts, printed grief material, engagement in community services and personal connections, the Donor Family Aftercare Advocate will support families through their healing journey. This position is expected to support all SC organ, eye and tissue donor families.

Essential Duties & Responsibilities

  • Plan annual calendar of aftercare engagement opportunities for donor families, including but not limited to: Annual Remembrance Day Ceremony, Honor Your Loved One Days, Grief Workshops, Candlelight Ceremonies, Donor Family picnics
  • Engage in support of donor families through regular phone calls and in-person meetings
  • Respond to donor family requests and inquiries in timely manner
  • Develop and oversee Donor Family Council, ensuring regular meetings and effective

follow-up through demonstrated action on council initiatives. Determine framework for DF Council, member selection, term limits and administrative roles.

  • Appropriately document and trend all calls, letters and communication with donor families in Electronic Medical Records, per SHSC standard procedures.
  • Support families through the written correspondence process with transplant recipients
  • Actively pursue professional development through trainings, workshops and conferences
  • Act as organizational liaison for donor families with regard to Funeral Homes and Coroners when necessary
  • Stay current with community grief resources and develop comprehensive library of support materials
  • Support donor families through coordination of donor family/recipient meetings and documentation, according to SHSC standards
  • Serve as liaison and advocate to donor families for community engagement and volunteer opportunities
  • Develop and distribute, with support from Administrative Assistant, surveys to donor families to ensure effective program structure
  • Coordinate national donor family opportunities such as Transplant Games of America and Donate Life Rose Parade
  • Other duties as assigned
  • BA Degree in Social Work, Psychology or related field;
  • Two (2) years of experience supporting individuals through grief and/or death and dying;
  • Grief Counselor certification preferred; or,
  • An equivalent combination of education and experience.
