What You Will Be Doing

Assist in the coordination of the social service programs of Senior Care Partners P.A.C.E. in accordance with our regulations and our established policies and procedures.

  • In-home comprehensive assessments and care planning.
  • Community resource management.
  • Collaboration with participant's family to provide support, and education.
  • Advance Care Planning experts.
  • Provide caregiver burden education.
  • Provide emotional support and assistance to Senior Care Partners P.A.C.E. participants in maintaining their independence for as long as possible.
  • Functions as a member of the Interdisciplinary Team (IDT), communicates participant changes to team members, participates in IDT meetings, and the development of participant plan of care.
  • Will demonstrate the ability to provide supervision requirements to Senior Care Partners P.A.C.E. LLMSWs and interns.
  • Documents in the participant’s medical record as required by policy.
  • Participates in Quality Improvement activities as needed.
  • Keeps leader informed of problems or issues.
  • Performs other duties as assigned.

What We Require

  • Must possess a master’s degree in social work (MSW) and be fully licensed in the State of Michigan.
  • Minimum of one year of experience working with the frail, elderly, or long-term care population.
  • Detail-oriented with good problem-solving skills.
  • Strong communication skills, both written and verbal.
  • Well-developed ability to show discretion and maintain confidentiality.
  • Excellent organizational skills.
  • Proficiency with MS Office Suite, Word, Outlook, PowerPoint & Excel.
  • Possess current CPR certification by AHA or willing and able to obtain as offered by Senior Care Partners PA.C.E.

  • Must provide exceptional customer service in support of all staff, external partners, and participants.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Immunizations and vaccines must be current or medical documentation must be provided from a medical provider with reason (s) to request waiver (Hepatitis B, TB, MMR, Varicella, TDaP, Flu, COVID-19).
  • Must have reliable transportation and be able to travel between centers as needed; current valid Michigan driver’s license and current valid vehicle insurance required.

What Will Be Required Physically (With or without accommodations)

  • Must be able to cope with the mental and emotional stress of the position.
  • Must function independently, and have flexibility, personal integrity, and the ability to work effectively with participants, personnel, and support agencies.
  • Must be able to push, pull, move, sit, or stand for long periods of time.
  • Lift up to 20 pounds as needed.

What We Offer

We offer a competitive benefits package including medical, dental, vision, Teledoc, HelpNet, and company paid life insurance and AD&D at $50,000 each, with the option to purchase additional voluntary insurances – STD, LTD, AI, CI, and additional life and AD&D. Our employees are eligible the first of the month following 30 days of service. All salaried staff receive 20 days of PTO accrued during the year, and non-exempt staff receive 18 days, in which is accrued right away. Employees may start contributing to our 403(b) immediately, and after one year of service the company will match up to 3%. After 90 days of employment full-time employees are eligible for an 8 hour floating holiday, and receive one each calendar year thereafter. All full-time employees receive the six major paid holidays.
