GENERAL SUMMARY
The Social Worker works in our beautiful Day Treatment Center located in Brighton, Michigan. They are a member of an interdisciplinary care team and address the psychosocial needs of clients with traumatic brain injuries and their families. The Social Worker conducts assessments, develops treatment plans and provides individual and group counseling. They assist clients and families in adjusting to the rehabilitation process and extended care living. This Mental Health Professional provides individual and group counseling and crisis management with clients and families. Additionally, they participate in internal case management to coordinate and maximize outcomes of rehabilitation process.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Provides Remarkable Care Every Day!
2. Participates as a member of the interdisciplinary team to develop interdisciplinary care plans for clients served by that team.
3. Identifies medically-related social and emotional needs of clients and families, and develops and implements programs/ services to meet those needs including individual and/or group counseling.
4. Assists with admission and discharge procedures.
5. Documents as required by local, state, and federal rules and regulations and as needed to meet the needs of the interdisciplinary team, the client, and the family. Maintains confidential records for sensitive material not appropriate for the client’s chart.
6. Manages crisis intervention with clients and families as appropriate.
7. Provides information to clients and families regarding Advance Directives, Guidelines for Life Sustaining measures and Client Rights.
8. Provides client, family, and staff education and training as needed and documents appropriately.
9. Timely documentation of client assessments, progress notes, care plans, discharge summaries, and outcomes data in accordance with existing criteria.
10. Participates in Care Conferences and Team Meetings and communicates with members of the team concerning client treatment as necessary.
11. Maintains good rapport and cooperative relationships with team and approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participates in their resolution.
12. Maintains confidentiality of information pertaining to clients, staff, and business operations, and adheres to all HIPAA standards.
13. Behaves in accordance with the Mission and Values of Resilire Neuro.
REQUIRED EDUCATION, EXPERIENCE, AND LICENSURE
1. Degree from an accredited college or university.
2. Professional license as required by the State of Michigan.
3. Valid driver’s license.
