An established and rapidly growing internal medicine, primary care, and family medicine practice in Northwest Indiana is seeking a full-time Nurse Practitioner to join our practice to support our Crown Point , Valparaiso and Munster office sites. Our practice has provided, for the last two decades, the platform for APNs to provide value based care to the community, incorporate technology and actionable data to ensure patient safety and good patient outcome.

The nurse practitioner, in collaboration with the physician, will provide medical care and treatment for patients older than 18 years of age. The Nurse Practitioner must maintain knowledge of evidence-based medicine in treating chronic diseases and prevention and also maintain appropriate documentation and coding.

The nurse practitioner will also be part of a team that delivers evidence based weight management solution.

The practice works on an electronic health system and has a well-organized clinical flow with a team of efficient medical and billing professionals.

Full Time Position

Education -

· Master’s of Nursing, Family Nurse Practitioner

Requirements -

· Valid DEA

· Valid state of IN license

· Good interpersonal skills

· Attention to detail

· Minimum of 1 year experience preferred

· Experience in endocrinology and/or obesity medicine is a plus.

· Minimum of 1 year experience preferred
