Responsibilities:

• Assess patient's safety and functional performance needs in their home environment post surgery or other health incident in accordance to Dr's order.
• Create Plan of Care for patient's with various diagnosis (ortho, neuro, etc.) and effectively communicate treatments needs with COTA.
• Use a variety of evidence -based methods to improve a person's functional mobility, safety, and independence to perform daily living activities.
• Explain the use of assistive devices and adaptive equipment to patient and family.
• Educate patient and family on home safety techniques
• Complete online documentation
• Supervise COTA
