Job Title: Outpatient Licensed Master’s Social Worker
Reports to: Outpatient Team Supervisor
Department: Outpatient -Niles
Grade: Union - 13
Type: Exempt
POSITION SUMMARY:
Provides individual, group, and family therapy to individuals diagnosed with mental illness and substance use disorders. Prepares comprehensive Biopsychosocial assessment, treatment plans and periodic reviews. Completes brief assessment, crisis intervention and pre-screens as needed.
EDUCATION:
Master’s Degree in Social Work.
EXPERIENCE/KNOWLEDGE:
Two or more year's experience providing treatment to individuals with a mental illness. Experience in treating individuals with substance use disorders preferred. Excellent computer and interpersonal skills, punctuality, ability to organize and manage several projects/tasks simultaneously, and work independently.
LICENSURE/CERTIFICATIONS:
Licensed by the Michigan Department of Commerce or Professional Regulation as a Licensed Master’s Social Worker with a clinical designation.

GENERAL DUTIES AND RESPONSIBILITIES
· Performs duties in accordance with agency standards of conduct and ethics
· Complies with all applicable BMHA policies and procedures
· Present and prepared on time for work, meetings and events
· Participates in required trainings and supervision
· Maintains required licensure and/or certification
· Maintains confidentiality for all information regarding consumers and staff of BMHA
· Assist in assuring BMHA compliance with all related regulatory body requirements and accreditation requirements

POSITION DUTIES AND RESPONSIBILITIES:
· Performs assessments and completes documentation in a manner which meets payor, regulatory, or agency requirements
· Develops treatment plans with measurable goals and objectives that addresses consumer needs as identified through the assessment process which meet the payor, regulatory or agency requirements
· Demonstrates a working knowledge of population specific diagnosis and displays the ability to gather, interpret data, and provide treatment in a manner that is consistent with treatment standards and best practice guidelines
· Completes brief assessment, crisis intervention and pre-screens as deemed appropriate
· Completes the CAFAS/PECFAS, LOCUS or ASAM and Periodic Reviews
· Applies culturally competent Recovery based practices
· Provides a trauma informed culture of safety to aid individuals in their recovery process
· Implements person centered planning as a shared decision-making process with the individual, who defines their own life goals and is assisted in developing a unique path toward reaching the goals
· Implements treatment through various therapeutic modalities such as individual, family, and group therapies
· Refers and links consumers to appropriate service (internal/external) programs. Consults with other services/agencies around case-specific and programmatic issues
· Completes authorization requests in manner that assures appropriate services are authorized
· Ensures due diligence is provided and discharge summaries are completed in a manner which meets regulatory and/or agency requirements
· Performs other duties as assigned
KEY PERFORMANCE INDICATORS:
· Complete progress notes within one business day after service is rendered, 95% of the time
· Completes assessments within three business day after service is rendered, 95% of the time
· Completes treatment plans within the time frames set by payor, regulatory, or agency requirements, 100% of the time
· Provides face-to face billable services 65% of time worked annually
· Completes Period Reviews within the time frames per agency requirements, 100% of the time
· Completes the CAFAS/PECFAS, LOCUS, or ASAM within the time frames established by the agency, 100% of the time
· No more than 5 instances of services will be written off due to clinician error will occur annually
· Show rate will be 80% or higher for all clinical appointments, 90% of the time
