Occupational Therapist Job Description

Definition :
An Occupational Therapist (OT) evaluates and provides family oriented services to the pediatric home health patient with neurological, musculoskeletal, orthopedic, and cardio-pulmonary impairments; fine motor, sensory motor, and developmental delays, as well as traumatic injuries and swallowing disorders. This includes patient and caregiver education and consultation/care planning with internal and external providers.

Example of Responsibilities:

· As part of a comprehensive evaluation, assesses patients’ abilities and level of functioning, and coordinates treatments designed to achieve patient/family-centered goals.

· Makes home visits in accordance with established Plan of Care (POC).

· Conducts on-going assessments and makes revisions to POC as appropriate, reports progress toward goals to Case Manager.

· Engages in a variety of means of communication with patient/family and other care providers as needed for care coordination. For example, phone calls, emails, messaging within EMR.

· Educates patients and care providers about safe use and recommended methods for installation of adaptive equipment.

· Identifies barriers to optimal levels of patient functioning and works with care providers to overcome these barriers.

· Identifies environmental and safety hazards in the patient’s place of care and works with care providers to minimize risk of injury.

· Provides written and verbal instruction to caregivers related to exercises that can be performed with the patient to assist with continued progress toward goals.

· Completes documentation in a thorough and timely manner to optimize care and team communication.

· Advocates for the patient and family including IEP meeting collaboration, letters of medical necessity, and connection to community resources.

Skills:

· Mastery of the core of the practice and profession of OT as it is applied in the pediatric home care setting and as explained in the OTPF.

· Ability to integrate relevant evidence, literature, and epidemiological data related to the pediatric population.

· Demonstration of the ability to seek out new knowledge in order to meet patient needs as well as the demands of a dynamic profession.

· Organizational skills to maintain detailed treatment plans and effectively manage assigned case load.

· Problem-solving skills to develop innovative solutions to unique challenges.

· Critical thinking – Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems.

· Reliance on experience and excellent clinical judgment to plan and accomplish goals.

· Demonstration of the ability to function as a productive, positive part of the patient/family care team

Required Experience:

Occupational Therapist: 2 years
