About the job:
The Physical Therapist will provide clinical physical therapy evaluation and treatment for program participants, on acute, restorative or maintenance levels as needed by Day Center, Nursing Home or in-home setting. Integrates the physical therapy treatment plan into the Interdisciplinary Team plan of care. Assists in the ordering, inventory, distribution and maintenance of durable medical equipment for participants.
Pay: $110,000-$115,000

· Education: Be a graduate of a physical therapy curriculum.
· Current Physical Therapy license required.
· Must have a valid driver’s license and have means of transportation.

Essentials:
· Conducts an in-person initial comprehensive physical therapy assessment on all participants upon enrollment. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
· 2. Conducts an in-person physical therapy re-assessment minimally every six (6) months or as needed.
· 3. Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in, Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
· 4. Provides treatment to participants and implements physical therapy procedures consistent in achieving identified goals. Grades activities to facilitate goals. Confers with Interdisciplinary Team on participants care issues.
· 5. Evaluates participant’s needs. Follows-up with participants during treatment, explaining precautions to take. Discusses condition and treatment plans and recommends solutions to participants care problems with Interdisciplinary Team.
· 6. Assesses the need for adaptive Durable Medical Equipment (DME), specialized shoes and prosthesis followed by recommendations to Interdisciplinary Team. Receives authorization prior to purchases. Assesses all newly issued orthotic shoes, braces and prosthetics for appropriateness and proper fit.
· 7. Assists as liaison with Senior Community Care nursing home and assisted living staff regarding physical therapy needs of participants who are their residents.
· 8. Evaluates charts and reports on participant’s progress, documents physical therapy notes in the participant’s medical record according to Senior Community Care charting requirements.
· 9. Assists in recommending physical therapy policies and procedures concerning services provided. Maintains quality assurance program, safety, environmental and infection control.
· 10. Directs, supervises, performs initial and annual competencies, and evaluates the performance of the Physical Therapy Assistant.
· 11. Performs competencies on appropriate direct care staff as needed.
· 12. Ensures that the job responsibilities, authorities, and accountability of direct reports are defined and understood.
· 13. Assists in training and provides ongoing education and clinical supervision for Physical Therapist Assistant(s). Adheres to department policy and physical therapy practice laws on Physical Therapy Association practice and reports to direct supervisor regarding these issues.
· 14. Acts within the scope of his or her authority to practice.
· 15. Educates and instructs participants and family members regarding in-home exercise programs: gait training; transfers; Activity of Daily Living (ADL); home modifications; care and use of equipment.
· 16. Trains staff in body mechanics, use of equipment, transfers, gait patterns, and methods to maximize safety and functional abilities in performing ADL’s.
· 17. Assists with data collection and prepares reports. Maintains any necessary participant statistics.
· 18. Maintains department in a clean and orderly manner. Keeps equipment in good working condition according to manufacturers’ recommendations.
· 19. Follows all Senior Community Care policies and procedures, and Occupational Safety and Health Administration (OSHA) safety guidelines.
· 20. Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
· 21. Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.
· 22. Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
· 23. Participates in and supports Quality Improvement Initiatives.
· 24. May assist at Senior Community Care of Colorado PACE – Montrose as needed
· 25. Performs other duties as required.
