James River Hospice is actively seeking a compassionate and highly committed ONCALL Triage Nurse RN or LPN to join our growing team in Farmville, VA! You will be primarily responsible for providing after-hours nursing coverage. You will respond to telephone requests for assistance as well as make visits, when necessary, that include skilled assessment and interventions. You’ll join a team of dedicated professionals supporting patients with end-of-life compassion and high-quality care- a hallmark of our Hospice Nurses to ensure an optimal care experience for the patient!

Brief overview of Responsibilities:

• Coordinates hospice care by working with other team members as well as with the patients/family to ensure needs are met.
• Performs evaluations for hospice care along with informational visits as needed
• Takes responsibility for ensuring patients’ plan of care are kept current
• Prepares the care plan for Hospice Aide/ Home Attendant
• Prepares Clinical Notes and updates the primary physician
• Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required.
• Participates in on-call duties as appropriate
• Ensures that arrangements for equipment and other necessary items and services are available
• Other duties as assigned

Qualifications:

• Licensed RN or LPN authorized to practice in the Commonwealth of Virginia
• At least two years of experience with at least one year of experience working in hospice care preferred
• Licensed driver with reliable car for making home visits
• Experience with Homecare Homebase preferred, not required.
