RN Case Manager will provide skilled care for patients in their home. Home health care experience is imperative. Must be dependable and be able to work with little supervision. Skilled nurse coordinates patient's care with family & physicians.

Qualifications-

Current RN license in state of Indiana

Skilled Nurse must have excellent communication, follow up, & documentation skills.

RN must have good work history .

Home care experience is required.

Current Drivers License, vehicle insurance, and access to dependable transportation.
