Licensed Social Worker - Discharge Planner - Nursing Home Introduction:

To oversee, for all Short-Term residents, with input from the care team, resident and family, the development of the interdisciplinary care plan, coordinate facility and outpatient services and develop a realistic discharge plan.

Licensed Social Worker - Discharge Planner- Nursing Home Job Summary:

• Meet newly admitted residents and family. Complete or coordinate the completion of admission paperwork.
• Initiate plan for discharge upon admission and throughout stay in coordination with IDT.
• Develop initial plan of care with input from various disciplines, resident and family.
• Participate in ongoing evaluation of care plan at scheduled meetings and assure it is amended as necessary to insure that goals, problems and approaches are consistent with resident’s condition and covered by resident’s insurance plan.
• Coordinate activities of all clinical team members in accordance with the plan of care.
• Facilitate communication among care-givers and service providers.
• Assess resident’s discharge potential upon admission and throughout Marian Manor stay.
• Arrange for provision of services and equipment needed upon discharge.
• Assure that discharge documentation is completed prior to discharge.
• Attend weekly Short-Term Meeting to plan, coordinate and project continued stay requirements, resident goals and discharge plan.
• Participate in informal daily rounds to speak to and observe residents.
• Communicate regularly with family members/responsible parties to update them on resident progress.
• Document all communication in medical record.
• Complete appropriate section of MDS 3.0, Interviews, Care Area Assessments and Care Plan.

Licensed Social Worker - Discharge Planner - Nursing Home Qualifications:

• MA Social Worker license required.

• Proof of COVID19 Vaccination and Booster Required.

• Long-Term Care experience preferred.
• Must function independently in an organized fashion, have flexibility, personal integrity and the ability to work effectively with residents, fellow team members and support agencies.
• Must being available on a 24-hour basis as necessary.
• Must have excellent communications skills and the ability to deal tactfully with residents, families, facility staff, visitors, and government agencies.
• Must be aware of the importance of each department’s contribution to the resident’s plan of care and facilitate the team approach in all services provided.
