Purpose: To oversee the development, implementation, and maintenance of all aspects of care for patients with ventricular assist device in collaboration with the CT Surgery Medical Director; to coordinate and manage the patient care for this high-risk patient population throughout the continuum to optimize the patient's clinical, functional, and psychological status; to provide the communication link for coordination of care among the multidisciplinary health care team involved with this patient type; to work with the CT Surgery Medical Director in orchestrating a performance improvement program that focuses on continually improving the outcomes of these patients; to provide leadership and mentorship to the CT Surgery Program and Nursing Department through application of advanced clinical and experiential skills; and to facilitate participation in cardiac/CT surgery-related clinical research trials. Patient care includes care to patients in the following age groups: Adults (19 through 65) and geriatrics (65 years and above), though primary age group will be geriatrics.
Primary Functions- Job duties:
- Manage the care of the high-risk ventricular assist device (VAD) patient population across the continuum (acute care inpatient, within the home, physician office, clinic and extended care facilities) in collaboration with CT Surgeons, Heart Failure Cardiologist, and other members of the multidisciplinary team. This involves advanced assessment, planning and coordination of care; modification of the treatment plan as indicated; and the monitoring and evaluation of outcomes.
- Provide clinical consultation and education related to management and care of the complex cardiac surgical patient and VAD devices for staff and physicians as well as other disciplines who may have contact or care responsibilities for this population.
- In collaboration with the VAD medical Director, oversee performance improvement activities associated with VAD patients to improve patients' outcomes.
- Develop a therapeutic relationship with the VAD patient and family and maximize the patient's participation and control in their own recovery. Facilitate the access and utilization of community resources and organizations to assist in patient's recovery.
- Oversee and provide education to acute care staff (ED, OR, ICU, cardiac surgery stepdown).
- Teach and provide reinforcement for VAD patients and their families on how to care for themselves and provide feedback on their progress.
- Provide leadership skills such as time management, coordinating and meeting multiple patients and family needs and requests, meeting physician and team member expectations, and setting priorities and networking.
- Serve as the liaison and communication link between VAD patients and their families with the primary care providers, specialists, community organization, and patient care services.
- Coordinate and manage outpatient care for the VAD patient in the clinic settings and through home visitation.
Preoperative care:
- All preoperative assessments.
- Obtain all old records.
- Coordinate all pre-consideration testing, arrange for all consultations.
- Compile all data for VAD consideration and assist with the presentation of the data at weekly VAD meetings.
- Perform home inspections.
- Preoperative preparation/admission prior to surgery (including hemodynamic optimization).
- lntraoperative monitoring.
- Post-operative care while hospitalized.
- Leading multidisciplinary rounding for post-operative VAD patients to decrease the length of stay, potential avoidances of post-implantation complications.
- Perform dressing changes and monitor of the VAD wound.
- Daily maintenance and follow-up of equipment.
- Obtain prior authorizations for medication prior to discharge.
- The coordination of ongoing out-patient medical support required for the rest of these patient's lives (or until transplant).
- Substantial pre and post-operative education for VAD patients and their families.
Outpatient/post-implantation care:
- Provide nursing support in the outpatient VAD clinic.
- Visit lab and note preparation.
- Troubleshoot the daily issues that arise with this patient population (which includes medication issues, symptomatic issues and life issues linked to living with a VAD).
- Interaction with pharmacies and insurance companies to obtain authorization for testing.
General Responsibilities:
- Maintain the significant equipment par levels required for these patients.
- Arrange for a safe home environment for VAD patients by communicating the presence of these patients in the community to the electric company and local firehouses in case of emergency.
- Establish SBM as a VAD resource for Long Island, provide highly specialized services to VAD patients implanted at other hospitals who not infrequently end up in our ER where they need to be assessed, stabilized and then transferred when it is safe to do so, or as evidenced during Hurricane Sandy and the 2013 snowstorm, when several VAD patients came to SBUH to provide a safe environment in case they lost power at home. The coordinator assumes responsibility for these patients until they are transferred to their implanting hospitals or can safely go home.
- Essential member of the team that provides outreach to Community Cardiologists and practitioners to advertise our VAD program.
- Primary educator for EMS services on Long Island for the care of these patients.
- Run an outpatient VAD support group.
Call:
- Takes 24 hour call/7 days a week for these patients, always being available for this group of very high acuity-patients.
Data Analysis and Reporting:
- Responsible for the extensive amount of data collection and entry, required for continuation of the program according to regulatory requirements, into the INTERMACS registry which tracks VAD patients post-operatively.
- Responsible for safety data collection and presentation to the monthly Heart Failure CQI meeting.
Qualifications
REQUIRED QUALIFICATIONS:
- Advance provider, a Nurse Practitioner with a minimum of 3 years of critical care experience or equivalent.
- NPs must have current prescriptive privileges, including DEA certificate.
- Masters degree in Nursing.
- CPR / ACLS / BLS certification.
- Must possess excellent oral, verbal and written communication skills; customer service/ interpersonal skills, problem solving, organizational skills, time management skills and knowledge of relevant hardware and software.
- National Certification as a Nurse Practitioner.
- Graduate of an accredited Nurse Practitioner program.
- 4 days per Week, 10 hours per shift.
- On Call required. May work all shifts and weekends.
PREFERRED QUALIFICATIONS: DNP preferred. Experience caring for VAD patients. Previous teaching experience and involvement in community organizations.
